Residents of California and Virginia may exercise their rights under state law to request: information about the categories of personal information we collect, use, and disclose; specific pieces of personal information we process about them; correction of inaccurate personal information; and deletion of personal information.

Please note that if you are requesting deletion of your personal information, we encourage you to submit your request to our data partner Experian to ensure that we do not subsequently receive your information in a refreshed data file. You may submit a request to Experian here.

If you are submitting this request on behalf of someone else, please submit evidence of your authority to act on another's behalf (such as a signed authorization from the consumer or a power of attorney) as an attachment to this form.

We are required to verify your identity to begin processing your request. The information you provide may be shared with a third-party to verify your identity. By submitting this form, you acknowledge and accept this. Please provide all information that may be useful for the purpose of identifying you in various databases. If you utilize more than one Alternate Name (example: Legal First name is Thomas, Alternate First names are Tom & Tommy), please submit multiple requests, one request for each Alternate name used. Include your legal First name as shown on your government issued identification provided with each request.
